Required Steps Before Scheduling Your Appointment

If your card is not expired/expiring (if your card was lost or stolen), complete these steps to apply for a duplicate.

  1. Pre-Apply on the State of MN website
    *Completing the Pre-Application does NOT book your appointment. The pre-application link above will open the MN DVS site in a new window. The pre-application will open the MN DVS site in a new window. After completing the online pre-application, you may close the DVS window to return to this screen.
     
  2. No Name or Address Change: no documents are required.
     
  3. Name Change - REAL ID or Enhanced Cards: You will need to provide proof of name change. Accepted Name Change Documents
     
  4. Address change - REAL ID: You will need proof of residency. Accepted Proof of Residency Documents for REAL ID
     
  5. Address change - Enhanced Cards: You will need proof of residency. Accepted Proof of Residency Documents for Enhanced Cards
     

Multiple Transactions: Only one transaction per appointment. If you need to complete more than one transaction during your appointment, choose Multiple Transactions instead.
